                             - 2-Hour Medical Error Prevention Course

                             - 2-Hour Domestic Violence Course**

                             - 3-Hour HIV/AIDS Course **

** If these 2 courses are not completed prior to initial licensure,

they must be completed within 6 months of licensure

LICENSE RENEWAL for LCSW, LMFT, LMHC

                           DUE BY MARCH 31, 2013:

Required:  A total of 30 ceus which must include:

                               - 2-Hour Medical Error Prevention Course

                               - 3-Hour Ethics and Boundaries Course

                               - 2-Hour Domestic Violence Course every 6 years

     -  All courses Board approved and all offered online

     -  These are 'word' documents & can be downloaded to your   

         computer

     -  Take at your leisure - logon and log off as often as you wish

     -   Print your own certificates for all courses except the Laws & Rules

         Course.  The Board requires the original certificate.  It will be sent

         via US MAIL to your address.  You are responsible to get it to the

         Board

     -  Courses in Initial License Package may be taken individually

     -  For Course Fees, Please go to 'All Courses' in menu

        at top of this home page

     -  Certificates are normally mailed within 1 business day; however, in

        case of illness or vacation, all certificates will definitely be mailed

        within 15 business days

     -  All courses are followed by a test.  Should you fail the test, no one

        will know except you and me - you could re-take it a million times

        and never pay another cent!

     -  You must complete the evaluation in order to obtain credit for the

         course and be able to print your certificate!!!  Be sure to follow all

         instructions!!!

     -  Paypal is the mechanism we use to receive credit card payment - 

        you don't  need to have Paypal yourself. If you have any trouble

     -  If you were already licensed in 2006, the DV requirement is

        due within 6 years (2012) and every 6 years thereafter
